RN - Resident Care Coordinator

General Description:

The Resident Care Coordinator works under the guidance of the Director of Nursing to assess, plan, coordinate, and evaluate resident care. The Resident Care Coordinator will serve as a team member to promote the optimal level of functioning and the dignity of each resident.

Reports to: Director of Nursing

Qualifications:

  • Current license as a Registered Nurse in the state of Minnesota.

  • Current C.P.R. certification.

  • Experience in geriatric nursing preferred.

  • Knowledge of the MDS process preferred.

  • Must possess good written and verbal communication skills.

  • Must qualify for employment after criminal background study, per guidelines of the Minnesota Department of Human Services.

Responsibilities:

  • Participate in resident admission process including the completion of the admission assessment, review of physician orders, and initiation of nursing care plan.

  • Complete the Resident Assessment Instrument Process (MDS) to identify the unique needs of each resident upon admission, when significant changes in condition occur, quarterly, and annually.

  • Involve the Interdisciplinary Care Team and the resident’s family in the care planning process upon admission, quarterly, annually and upon significant changes.

  • Develop nursing care plans that are consistent with the physician’s orders and promote the optimal level of functioning and dignity of the resident.

  • Evaluate resident cares to determine if desired outcomes are occurring

         and modify the written nursing care plan as needed to meet desired outcomes.

  • Maintain positive relationships with families, communicating changes in resident

    status and facilitating family involvement in resident cares.

  • Provide nursing assessments and services as appropriate to each resident’s plan of care.

  • Work with Social Services to coordinate discharge planning.

  • Be available to charge nurses for consultation regarding needs of the residents.

  • Promote cooperation and teamwork by being a role model to all staff; promote positive relationships with other departments.

  • Participates in planning staffing ratios to best meet the needs of each nursing station and the facility as a whole.

  • Communicates with staff to develop and enhance performance. Promote retention of staff by affirming their individual contributions to the organization.

  • Demonstrate professional growth by actively pursuing continuing educational opportunities.

  • Identify staff educational needs and participate in providing ongoing education and orientation of staff.

  • Participates in quality improvement efforts.

  • To adhere to the Code of Conduct and Compliance policies as defined in the new employee orientation information.

Physical Requirements:

  • Ability to occasionally bend, stoop, squat, reach above shoulder level, crouch, kneel, and push/pull.

  • Ability to carry up to 35 pounds.

  • Ability to lift up to 75 pounds.

  • Ability to understand and use good body mechanics.
